Follicular-phase vaccination coincided with increased reports of fever, fatigue and local reactions, implying that fluctuating estradiol levels and follicle-stimulating hormone may shape both innate immunity and adaptive responses such as antibody production. Luteal-phase vaccination, under higher progesterone, tracked with fewer logged complaints yet a shorter interval to later infection in app records, hinting at a trade-off between tolerability and immunogenicity in real-world behavior data.
The more unsettling implication is that public-health guidance has largely ignored this dimension of biological timing. Clue’s longitudinal logs, while inherently noisy and dependent on self-report, provide a rare coupling of ovulatory status, reactogenicity profiles and infection outcomes outside a controlled trial. Researchers caution that confounding factors and selection bias remain significant, but the signal is strong enough to raise a practical question for clinicians and users of menstrual tracking tools: not only whether to vaccinate, but when in the cycle the needle meets the arm.
