The sharper message is that artificial sweeteners have not earned a clean bill of metabolic health. The association does not prove causation, and no specific biological pathway such as altered insulin sensitivity or changes in gut microbiota was tested within this synthesis, yet the consistency of risk across large populations will pressure guideline committees to treat these additives as a hypothesis that demands targeted randomized trials rather than a harmless default swap for sugar.
