The promise is provocative. Ketogenic therapy may alter metabolic signaling, neuroinflammation, oxidative stress, and neurotransmitter activity tied to post-traumatic stress disorder, according to an integrative review. The biology has a logic. Ketone bodies can shift neuronal energy use and may influence GABA and glutamate balance, mechanisms implicated in fear processing and stress regulation.

The evidence is thin. Animal experiments and a handful of human case reports, small cohorts, and pilot studies have recorded early hints of symptom relief, yet none can settle whether the diet itself drove those changes. Correlation wears a persuasive coat. Diet adherence, placebo effects, concurrent treatment, and selection bias can all distort results when participant numbers are low and follow-up is limited.
Restraint is the wiser posture. PTSD is not a metabolic switch waiting to be flipped, even if mitochondrial function and inflammatory signaling offer plausible targets for intervention. Controlled trials must test safety, durability, and clinical efficacy before ketogenic therapy earns a place beside established care. For now, the ketone signal glows at the edge of the laboratory: visible, suggestive, and still far from a verdict.
